Freshman Admission Requirements:
Admission Requirements - Freshmen: During Application: High School card not necessary. Submit the accomplished Application Form to the Admissions Office. Pay processing fee of P220, medical exam fee of P500 (for all programs except for Maritime Education) and minimum fee of P1,500. To Validate Enrollment: After high school graduation, submit: A. Original High School Card, F-138 B. Original copy of Certificate of Good Moral Character C. 3 Identical Photos, 2" x 2" D. Photocopy each of Diploma and PSA (NSO) Birth Certificate
Transferees Admission Requirements:
Admission Requirements - Transferees: For Evaluation (credits subjects from previous school), submit any one of the following documents: A. Scholastic Records B. Certificate of Grades C. Class Cards Requirements for Enrollment: A. Honorable Dismissal/Transcript of Records or Transfer Credentials B. Two (2) recent identical sutdio photos with white background C. Clear photocopy of PSA (NSO) Birth Certificate
Foreign Students Admission Requirements:
Admission Requirements - Foreign Students: 1. Authenticated Transcript of Records 2. Authenticated College Diploma 3. Authenticated Letter of Financial Support (2Copies) 4. Authenticated Police Clearance from Country of Origin 5. Authenticated Bank Statement 6. CEA (Certificate of Eligibility for Admissions) 7. Photocopy of Passport 8. Personal History Sheets from Philippine Embassy (8 copies) 9. Letter of Recommendation 10. 8 Copies 2”x2” Picture 11. Foreign Fee Donation 12. Certificate of Good Moral Character 13. Birth Certificate 14. Application for Admission 15. Alien Certificate of Registration 16. Students Visa and Study Permit 17. NMAT (National Medical Admissions Test) (For College of Medicine students only) IMPORTANT: All papers must be authenticated by the Philippine Embassy.
