Photocopy of Certificate of Good Moral Character 6. Photocopy of Birth Certificate 7. 1 clearance (either of the ff: NBI, Police, Baranggay Clearance) 8. Interview with School Director 9. Medical Exam (urinalysis, fecalysis and chest x-ray)

Transferees Admission Requirements:
