Clear photocopy of authenticated PSA/NSO Birth Certificate 4. Original Medical Certificate issued by any government hospital 5. Two (2) copies of identical picture with specifications, as follows: • Passport Size (3.5 x 4.5 cm) • Colored, with white background • With computerized applicant’s name in the format: First Name, Middle Initial, Last Name and Name Extension (if any) 6. Two (2) Long Brown Envelopes

Transferees Admission Requirements:
