Freshman Admission Requirements:
Admission Requirements - Freshmen: For Senior High School (SHS) applicants: Qualifications: 1. Finished Grade 10 Junior High School (for Grade 11 applicants) 2. Finished Grade 11 Junior High School (for Grade 12 applicants Requirements: 1. Application form (get the form here: https://www.mfi.org.ph/wp-content/uploads/2014/09/Application-Form.pdf) 2. Two (2) pieces 2×2 ID photo (white background) 3. Certificate of Good Moral Character 4. Form 138 – Grade 10 report card 5. Certificate of Grade 10 completion 6.Certificate of ESC voucher recipient (from students coming from ESC schools) 7. QVR (for qualified voucher applicants only) from DepEd For Industrial Technician Program: Qualifications: 1. At least a High School graduate – second coursers and current college students are welcome 2. No age limit Requirements: 1. For High School graduates, photocopy of Secondary report card (Form 137) and High School diploma 2. For High School graduates of Department of Education’s Alternative Learning System (ALS), photocopy of the Certificate of Completion 3. For transferees, school records or transfer credentials from last school attended. If you are admitted, submit copy of grades and subject descriptions should you wish credit units. 4.
