Freshman Admission Requirements:
Admission Requirements - Freshmen: For Entrance Exam 1. Photocopy of your birth certificate issued by the National Statistics Office 2. Photocopy of current school ID 3. One (1) 2x2" ID photo 4. P350 entrance exam fee For Enrollment 1. Original copy of Form 138 (Fourth-year high school report card or it's equivalent) 2. Original copy of Certificate of Good Moral Character from your class adviser, guidance counselor or principal 3. Three (3) 2X2" ID photos 4. Enrollment application form 5. Foreign Students Admission Requirements:
Admission Requirements - Foreign Students: 1. Duly accomplished Application Form 2. Copy of Birth Certificate 3. Good Moral Character Certificate 4. Transcript of Record/Scholastic Records duly authenticated by the Philippine Embassy/Consulate located in the student-applicant’s country of origin or place of legal residence 5. Scholastic Aptitude Test (SAT) Results, in lieu of taking the Admission test 6. Photocopy of the data page on the student’s passport (i.e. Photo, date and place of birth, stamp of the latest arrival in the Philippines.) The passport itself shall be presented to the Bureau of Immigration for verification. Note: Please present original passport for verification 7. Copy of the Quarantine Medical Examination by the National Quarantine Office 8. IELTS Test Report Form with a minimum band score of 6 Note: US Citizen Students are not required to take IELTS exam but a certificate/letter of English proficiency from the last school attended should be submitted.
