Recommendation from (http://www.febias.edu.ph/images/downloads/Recommendations-Teacher-Pastor-and-Friend.pdf) - Pastor - Teacher - Christian Friend 3. Scholastic records and Certifications a. For transferees: - Transcript of Records (TOR) - Certificate of Honorable Dismissal/Transfer Credential b. For High School graduates - Form 137-A or Form 138 - Certificate of Good Moral Character 4. Original PSA (NSO) Birth Certificate 5. Written guarantee of financial support from parents or sponsors 6. Health Requirements - X-ray result with big film (taken within six months) - Medical results of blood, urine, stool and drug test 7. Application Fee (P250.00) 8. Diagnostic Test (Diagnostic Test Fee P250.00) 9. Interview (Guidance Counselor & Resident Supervisor)
