Honorable Dismissal / Transfer Credential Certificate 5. And photocopy of Summary of Scholastic Records 6. Course Description (for crediting of subjects) 7. Transferee Recommendation Form

Foreign Students Admission Requirements:
Admission Requirements - Foreign Students: 1. Two pieces 2×2 colored ID Picture 2. Current passport pages where personal information and other details appear (with recent arrival stamp) 3. Transcript of Records or Certified True Copy of Grades authenticated by the Philippine Foreign Service Post in his/her country of origin with English Translation 4. Police Clearance authenticated by the Philippine Foreign Post in his/her country of origin 5. No Objection from the previous school (if applicable) 6. ICARD 7. Certificate of Good Moral Character 8. Authenticated Copy of Course Description (if local transferee) 9. Medical Certificate (if local transferee)
