Transferees Admission Requirements:

Foreign Students Admission Requirements:
Admission Requirements - Foreign Students: Freshmen 1. Notice of Acceptance (NOA) with the school official dry seal, addressed to: - The Director - Visa Division – Office of Consular Affairs Department of Foreign Affairs - Students Visa issued by Philippine Foreign Service (for more than 1 year or bachelor degree courses) - Study Permit issued by BID (for special course or less than 1 year courses) - Authenticated photocopies of Students Visa, ACR, Residence Certificate - Five (5) copies of the 1998 Revised Personal History Statement (PHS), duly accomplished and signed by the applicat in English and in national alphabet, accompanied by personal seal of any original left and right hand prints on PHS and original photo High School Transcript of Records/Scholastics Records, duly authenticated by the Philippine Embassy or Consulate in the applicant’s country of origin or legal resident “SEEN and NOTED” stamp is not acceptable. A notarized affidavit of support and proof of adequate accommodation and subsistence school dues and other incidental expenses. Photocopy of passport pages where name, photo, birth date and birth place appear. Transferee 1. Students Visa issued by Philippine Foreign Services (for more than 1 year or bachelor degree courses) 2. Authenticated photocopies of Students Visa, ACR, Residence Certificate 3. Certified True Copy of Certificate of Eligibility for Admission for students of Dentistry & Medicine 4. Certificate from the former school that they interpose no object to the student’s transfer to DOMC 5. Transfer Credentials/Honorable Dismissal 6. Transcript of Records/Scholastic Records 7. Two Certificate of Good Moral Character 8. NBI/Police Clearance 9. Four picture passport size

Foreign Second Admission Requirements:
Admission Requirements - Second Coursers: Admission into the College is selective. Any student who seeks acceptance should meet the following requirements favorably: A. Evaluation of the applicant’s past academic performance. B. Successful passing of DOMC entrance examination C. Submission of the following: 1. Transcript of records 2.
