One (1) piece brown envelope 8. Two (2) pieces of Documentary Stamp 9. Photocopy of National Statistics Office (NSO) Marriage Certificate for married students 10. Photocopy of voter’s Identification (ID) Card or Certificate from Commission on Election (COMELEC) of the student/student’s spouse/ student/student’s parents / student/student’s brother or sister for Malabon City Residents. 11. Drug test in any government hospitals

Transferees Admission Requirements:
Admission Requirements - Transferees: 1. Certificate of Transfer (Honorable Dismissal) 2. Transcript of Records 3. PSA (formerly NSO) Issued Birth Certificate 4. Certificate of Good Moral Character 5. Course Credit Application Form 6. For Filipino students who studied abroad, a copy of the Transcript of Records/Scholastic Records duly authenticated by the Philippine Foreign Service Post (PFSP)/PhilippineEmbassy in their country of origin must be submitted. 7.
