Accomplished Personal History Statement 4. Certificate of Bank Deposits 5. Affidavit of Support 6. Birth Certificate of Passport 7. Transcript of Records 8. High School Card 9. Seven (7) Copies of 2x2 Pictures 10. Two (2) Copies of 1x1 Pictures (Items 3-10 are needed for Student Visa Application) 11.
